Try to change your exercise routine around and don't continue doing the same thing over and over again. In order for you to see improvements you must stress the body so it can adapt and make changes to deal with the stress. Lifting a little bit of a lighter weight, but doing more repetitions.
You could lift heavier and do less repetitions.
If you use the treadmill, try to run for an extra 5 minutes, or run on an incline.
If you use the pool, swim holiday a float and just use your legs or hold a float between your legs and just use your arms.
